Lori Franklin, RN, RAC-C

Regional Director, Care Coordination 

Lori Franklin is a Registered Nurse and healthcare leader with more than 25 years of experience in case management, care coordination, post-acute care, and population health across a variety of healthcare settings. Her experience includes acute care hospitals, inpatient rehabilitation facilities, skilled nursing facilities, hospice, and Accountable Care Organizations.

Throughout her career, Lori has been committed to improving care transitions and helping patients receive the right care, in the right place, at the right time. Since 2023, she has worked closely in partnership with Alliant Management Services while serving at Northwest Florida Community Hospital, collaborating with rural hospitals across Northwest Florida to strengthen regional swing bed programs and expand swing bed utilization.

In her role as Regional Director of Care Coordination, Lori now brings that successful partnership directly to the Alliant team. She works alongside hospital leadership, providers, case management teams, and community partners to improve patient outcomes, enhance access to post-acute services, and support the long-term sustainability of rural healthcare organizations.

Lori has helped rural hospitals achieve significant growth in swing bed admissions, patient days, and program sustainability through strategic care coordination initiatives and relationship-building with referral partners. Her leadership is rooted in a patient-centered philosophy and a deep understanding of the challenges and opportunities facing rural healthcare.

Having lived and worked in a small-town community herself, Lori understands the vital role rural hospitals play in the health and well-being of the communities they serve. She is passionate about helping these organizations thrive while ensuring patients have access to high-quality care close to home.
